Privacy

You have a right to expect as much visual and auditory privacy as may be reasonably provided given the limitations of our facilities. Should you have concerns in this regard, you are asked to speak to your counsellor. Also, invoices for services may be mailed in unmarked envelopes and caller id blocking may be used for calls to your home at your request.

Confidentiality

Under normal and regular circumstances, all counselling related information will be treated as confidential and will not be shared with a third party without your signed and informed consent. Under some circumstances, all professionals are legally and ethically bound and free to disclose certain specific and relevant information to appropriate individuals or agencies. These circumstances include the following:

  • If there is reason to believe that the client is a danger to himself or others;
  • If there is reason to believe that the client may do harm to another person;
  • If the client acts in an abusive manner towards the counsellor
  • If there is reason to believe that a minor has, is or may be in danger of neglect or abuse;
  • In response to a court order, or
  • In response to litigation initiated by a client against a counsellor.

Informed Consent

After sufficient time to formulate an appropriate assessment, your counsellor will develop, in discussion with you, a counselling plan.

Client Records

All counselling related material is the property of Christian Counselling Centre. It will be securely and confidentially maintained for a period of ten (10) years after the completion of the last counselling session after which time, under normal circumstances, it will be destroyed in a complete and responsible manner.

You have the right to review any information that is in your client file, and this will be done in the presence of the PHIPA designate. Under some circumstances, you may be able to make copies of information on file upon providing a written request. Under no circumstances will you be able to review documents that have been prepared by other professionals and shared with the counsellor to assist in planning counselling. This information can be reviewed only with the professional or the agency, which initially generated the information.

Client Satisfaction Questionnaires

At predetermined times, the Administrator will distribute Client Information Questionnaires which will provide an opportunity for you to comment on the service received or being received. Information from this process will be collated and will be interpreted to the board by the Executive Director. At all times, your privacy will be respected and any identifying information will be removed. Ideally, your counsellor will not be able to identify the source of the information you provide. This will ensure that you will be able to express yourself honestly without concern for repercussion.
